We are Springfield Healthcare in Hull, and we provide care and support in people's own homes across the city. We support older people, adults with learning disabilities and autistic people, people with physical disabilities and younger adults. Our work divides roughly into two kinds. For older people it is usually about reliable help at the harder points of the day, so that somebody can carry on living in their own home rather than moving somewhere unfamiliar. Support is planned with the person, in their own words, alongside family and professionals where they want them involved, and we review it as circumstances change.
