We are Humber Healthcare, a home care agency supporting adults across Hull. We visit people in their own homes so that they can stay in the place, and among the people, they know best. The support we provide covers a wide range of needs. We work with older people and with people living with dementia, with adults who have learning disabilities or autism, with people living with mental health conditions, with people who have physical disabilities or a visual or hearing impairment, and with younger adults who want help to live on their own terms. What we offer runs from a short daily call through to several visits a day covering personal care, medication, meals and company. We start by listening. People usually know exactly what they want help with and what they would rather keep doing themselves, and a care plan that ignores that will not last a fortnight. We agree the plan together, keep to the times we promise, and record what we do so that any change in someone's health is picked up quickly. Our most recent overall rating from the Care Quality Commission is good. We are proud of it without treating it as the end of the matter.
