The job is to keep a household going so that somebody can stay where they want to be, and most of it is practical. Getting a person up and washed without leaving them feeling handled. Help with dressing, which sounds minor until buttons and shoes become impossible. Medication at the right time and properly recorded. A meal actually cooked. Shopping, laundry and a tidy kitchen. Help to move safely about a flat that was never designed for somebody who struggles to get about. For people living with dementia we keep the same small group of carers going to the same homes, because familiarity is what makes a visit calm and a familiar carer is the one who spots trouble early. For people with mental health needs, what usually helps is structure and a presence that does not vanish when a fortnight goes badly. London traffic being what it is, we plan rounds that can actually be achieved rather than ones that look efficient on paper. Being late is a failure rather than an inconvenience.
