Plans are written with the person, reviewed honestly and changed when they stop fitting. We also work alongside families and with social work and health colleagues, since joined up support is what tends to last. Our most recent overall rating from the Care Quality Commission is good. We keep examining our own practice, particularly around choice and independence.
