About Reablement Service
We are the reablement team for Cheshire West and Chester, working with people in Ellesmere Port and across the borough. Our purpose is a little different from ongoing home care. We are there for a period of time, usually after a hospital stay, an illness or a fall, to help someone get back to doing things for themselves. That means our success is measured by how little we are needed at the end. We work alongside people rather than doing things for them, building confidence and strength back up step by step, practising the everyday tasks that matter most, and adapting how something is done when the old way is no longer possible. We support older people, including those living with dementia, alongside adults with a physical disability, people with a visual or hearing impairment, people managing a mental health condition and younger adults. Support can involve personal care, medication, meals, moving about safely at home, and regaining the confidence to get out again. We work closely with therapists, community nurses, social workers and families, because reablement only works when everyone is pulling the same way.
