Something much more substantial where that is what makes staying at home possible. We agree it with the person, write it down plainly and adjust it as things change rather than at fixed intervals. London can be an isolating place to grow old in, and for some of the people we visit we are the main regular contact in the week. We treat that as part of the job, not an extra.
