Because the people we visit differ so much from one another, we do not work to a template. We begin with a conversation in the person's own home about what has become difficult, what they would rather keep doing themselves, and what would actually make the week easier. From that we agree a plan, and we change it as needs change rather than waiting for a review date. Our visits cover personal care, prompting and support with medication, meals, shopping and housework, and support to get out to appointments, work or simply to see people. We keep the group of carers around each household small and familiar, because personal care is far easier to accept from somebody you know. Carers are asked to notice as well as to do, and to raise anything that worries them with us on the day so that we can involve a relative or a GP early.
