About Choices for Living Well (Reablement)
Choices for Living Well is a reablement and home care team working across Bury. Reablement is the heart of what we do, and it is worth explaining what we mean by it. Rather than taking tasks over, we work alongside people for a short period so that they can take those tasks back. Someone coming out of hospital unsteady on their feet may need help with washing and cooking in the first week, less in the second, and by the fourth may well be managing again alone. That is the outcome we aim at. We support older adults, younger adults, people living with physical disabilities and people with loss of sight or hearing. We also support people living with dementia, where the goal shifts a little. There we look to hold on to the skills and routines someone still has for as long as possible, and to keep home feeling like home. Where ongoing support is what is genuinely needed, we provide that too, and we say so honestly rather than pretending independence is always around the corner. What we will not do is create a dependence that nobody asked for. Everything begins with a conversation in the person's own home about what they would like to be able to do again. If you or someone you care for is coming home from hospital, or finding daily life harder than it used to be, please contact us.
